Ovum Donor Program Costs and Payment Options

IARC®'S FINANCIAL RESPONSIBILITIES:

THE FOLLOWING EXPENSES ARE USUALLY INCLUDED, DEPENDING ON THE CLINIC YOU SELECT, AND WILL BE PAID AS PART OF OUR FLAT FEE PROGRAM OPTION:

• The ovum donor's reimbursement, if applicable;
• The ovum donor's medical and psychosocial screening (MMPI2 and full psychological testing are extra);
• The ovum donor's medication;
• The ovum donor's travel, accommodation, and meals during fertility treatment for the egg retrieval cycle;
• The ovum donor's child care and other reimbursable expenses during fertility treatment for the egg retrieval cycle;
• The ovum donor's accidental insurance policy to cover complications arising from the egg retrieval and embryo transfer;
• All physician, embryologist, hospital fees, and usual medical expenses for the IVF/egg retrieval performed for the donor and the couple at the fertility center;
• All lab, blood and ultrasound requirements during cycle monitoring egg retrieval for the client and donor;
• Cryopreservation of embryos;
• Embryo Storage (1 year, then subject to annual fees);
• Embryo transfer costs for all embryos produced from the ovum donor's retrieval; and
• Sperm Preparation and storage.

CLIENT'S FINANCIAL RESPONSIBILITIES:
THESE COSTS WILL BE PAID BY THE CLIENT DIRECTLY WITHOUT THE USE OF IARC®'S EXPENSE ESCROW ACCOUNT:

• Client's airfare and hotel/travel expenses, if necessary;
• Client's medical testing;
• Client's medication;
• Intra-cytoplasmic sperm injection (ICSI), blastocyst culturing, assisted hatching, and immunoglobulin therapy, if required; and
• In the case of medical complications, all costs not covered by the donor/recipient's medical or accidental insurance.

COST OF OVUM DONOR PROGRAM

IARC® usually charges a flat fee that covers all of the ovum donor's expenses related to one cycle, although we will arrange other payment options, if preferred. The flat fee covers all of the services included above and varies depending on the clinic and program option you select. The flat fee prevents financial surprises and allows you to budget. Please be careful. Many fertility centers may seem to have lower fees, but there are extra fees for the physicians, hospital, anesthesia, cryopreservation, and lab tests. Some of IARC®'s program options include the cryopreservation, one year's storage, and subsequent embryo transfers of the frozen embryos. Your stored embryos can be used in the future for subsequent attempts. If you have successfully had one child but still have frozen embryos remaining, you are welcome to return to your clinic for another genetically-related child after the first birth. (There is an additional fee charged by the clinic in this case.)

PAYMENT SCHEDULE

THE FEE MAY BE BROKEN INTO TWO PARTS:

To view Ovum Donor files prior to signing an agency agreement with IARC®: $500 U.S.
(Effective for 6 months. If you have not selected an ovum donor within this time period, you will need to pay another $500 to review more profiles.)

Upon signing legal contract with IARC® for the Ovum Donor Program: Remaining Balance.

IARC® appreciates that this is a large amount of money to make available prior to starting with your donor. Please note that once you sign up for the program, we start immediately to match you with a donor of your choice. Once matched, the program progresses quickly and can usually be completed within 3 months' time. All expenses are paid out during that time. This is why we need funds available at the commencement of the program.

Please note: Provisions can be made if the client's medical insurance plan covers some of the expenses.

You may make payments to IARC® by credit card or wire transfer.

Please contact us for wire transfer information when you are ready to start your program.
(Please receive a written confirmation of your wire transfer from your bank and fax it to IARC®'s Accounting Department immediately whenever a wire transfer is made.)
